We’re seeking a dedicated Community Support Worker to provide personalised support within the Consumer Directed Care model at Eyre and Far North LHN. In this role, you will assist aged, frail, or disabled clients with personal care, medication support, cleaning, meal preparation, transport, and social support. You’ll work closely with consumers who actively choose the services they receive, how often, and who provides them—helping them maintain comfort and independence in their own homes.
