About the Role: We are seeking a passionate Physiotherapist to make a meaningful difference in patients' lives through our innovative Northern Reablement and Intermediate Care to return Home (NRICH) service. In this role, you'll work within a dynamic multidisciplinary team, supporting our transformative approach to healthcare delivery by helping patients regain independence and confidence as they transition from hospital to home. The Northern Reablement and Intermediate Care to Return Home (NRICH) Service is delivered in partnership with the Aged Care and Housing (ACH) Group, at the HEALTHIA Residential Aged Care Facility, Elizabeth Vale. ACH Group provides specialist aged care support for NRICH patients across 24 hours, 7 days per week. The NALHN Geriatric Service multidisciplinary team provides medical oversite, care planning and a reablement program to optimise functional independence in a home-based setting to support safe discharge to home. To be eligible for this position, you must hold an appropriate Degree or equivalent qualification which entitles registration with the Physiotherapy Board of Australia. About You: To thrive in this role, you will possess well‑developed skills in physiotherapy assessment and evidence‑based treatment techniques, along with proven competence in managing a diverse clinical caseload. You will show a commitment to ongoing professional growth through self‑directed learning, and demonstrate clear, confident communication in both verbal and written forms. Effective time management, and the ability to collaborate within inter‑disciplinary teams are essential. Experience using Microsoft Office programs such as Word and Excel, as well as relevant database systems, will support efficient documentation and workflow.
