Join our Rehabilitation, Aged & Palliative Care Team at SALHN
Provide essential clinical interventions and lead discharge planning for patients transitioning from acute care. Be involved in contributing to the reporting and quality monitoring mechanisms of Transition Services. We are seeking a dedicated Part-time Weekend Physiotherapist to join our Transition Care team.
About the role:
Reporting to the Transition Services Manager this role is responsible for contributing to the implementation and management of the Southern Adelaide Local Health Network Transition Services service delivery model. Duties for the role will include:
- Utilising specialised therapy skills to provide physiotherapy intervention and case management to patients
- Contributing to the development of an integrated team approach and culture which is highly responsive to the needs of our consumers.
- Working cooperatively within a multi-disciplinary team to continually improve, implement and evaluate a holistic Transition Services which is responsive to identified community needs
- Providing overall care co-ordination for clients within Transition Services through regular contact with service providers and the acute sector
- Act as a key contact for patient/family/carers during admission
About you:
- Appropriate Degree or equivalent qualification which entitles registration as a Physiotherapist with the Australian Health Practitioner Regulation Agency (AHPRA).
- Strong clinical Physiotherapy experience in working with older people and restorative programs in subacute and/or community settings
- Demonstrated experience with solution focussed problem solving in complex case management situations and participation in complex and timely discharge planning
- Ability to contribute to plans, develop, implement and evaluate community oriented, holistic therapeutic interventions appropriate for older people and their families
- Sound skills in building rapport and communicating with a wide range older adults, their carers /family, referrers, hospital staff and service providers
- Demonstrated competency in both verbal and written communication
