About the Role As a Placement and Home Brokerage Officer, you coordinate safe transitions from hospital to community care. You manage brokerage pathways, build strong partnerships and help older people access the right services for timely, supported discharge.
About You You are organised, adaptable and people focused, with strong communication skills and the ability to manage competing priorities while supporting safe, timely patient transitions.
